Thank you for volunteering your time this season for Tavistock Minor Hockey.
Vulnerable Sector Checks will now be collected by Ontario Hockey Federation OHF (instead of Tavistock minor hockey). If OHF does not receive your vulnerable sector check and declaration form, then your name will not be on the roster as a coach, assistant coach or manager or trainer.
** For the 2024-25 season, the OHF will be accepting copies of a Vulnerable Sector Check from 2022-23 or 2023-24 AND the OHF Screening Declaration Form (PDF).

If you have your Approved Vulnerable Sector Check, OHF Declaration form and HCR#, then please submit your documents to the OHF Screening Submission Portal. All documents must be in PDF format.

If you require an updated vulnerable sector check, you can request one online. Please download OHF letter requesting a vulnerable sector check and fill in the current date, and your full legal name*
